Fraxel Dual Laser Peel
For Discolorations, Brow Spots and Actinic Keratoses (Pre-cancers)
What is the Fraxel Dual Laser Peel (FDLP) and how does it differ from Fraxel re:store?
The FDLP is a Fraxel treatment using Thulium, a different wavelength of light. This treatment works in the upper layers of the skin to remove brown spots, melasma, brown discolorations and actinic keratosis (pre-cancers.) It also improves skin texture and tone. It does not penetrate as deeply into the skin as Fraxel re:store and therefore does not have as much swelling or redness.
How many FDLP’s are needed and how soon will I see results?
Usually only 1 or 2 treatments are needed, 1 month apart. Results are seen immediately after peeling. More treatments may be needed for certain skin types (see box below.)
Will FDLP improve my wrinkles or scars or tighten my skin?
FDLP improves tone and texture but it does not improve wrinkles and scars or tighten skin. FDLP is therefore often used in combination with Fraxel re:store laser treatments to treat wrinkles or scars and with Pelleve’ Lunchtime Lift treatments to tighten skin. These other procedures are usually performed on different visits.
Does insurance cover FDLP if it is used for actinic keratosis (pre-cancers)?
We do not bill insurance or Medicare for FDLP even when actinic keratoses are treated.
How should I prepare for my FDLP?
For 2 weeks prior to FDLP avoid retinoids, glycolic acids, self tanners and sun exposure plus wear an SPF 30 sunscreen daily. The treated areas cannot be tan. For olive, Asian and darker skin types, patients treating melasma and patients with a history of post-inflammatory hyperpigmentation we recommend using a skin bleaching product such as Obagi Clear or Elure for at least 4 weeks prior to treatment (see box below.) These are available for purchase in our office. Also, please let us know if you have a history of cold sores/herpes on your lips/face so we can prescribe a preventative medication.
What can I expect after my treatment?
The skin develops a bronzed appearance on the 2nd to 5th day and then peels, usually on the 5th to 7th day. Body areas may take longer to peel than faces and may appear red for several weeks after FDLP. Aggressive facial treatments may also appear red for several days or weeks after treatment. Make sure to wear sunscreens with at least SPF 30 whenever you are outdoors, after your treatment.
What are the possible side effects?
You may develop some redness and rarely swelling but usually only with more aggressive or non-facial treatments. You will peel, that is expected. If you have melasma or have a darker skin type, there is a small risk of developing brown spots from any laser treatment. That is why we take special precautions with this type of patient (see box below.)
Can I treat just a small area or individual brown spots?
In most cases we do not recommend treating just a small spot or area since there may be an obvious color difference between where the laser did and did not treat.
Is FDLP safe for all skin types?
FDLP is safe for all skin types but we have special instructions for darker skin types, patients with melasma and patients who have a history of post-inflammatory hyperpigmentation (see box below.)
Special Instructions for patients with:
- Melasma (blotchy brown patches on face, AKA mask of pregnancy)
- Asian, darker skin types and individuals who usually tan and rarely burn
- A history of post-inflammatory hyperpigmentation (skin that has darkened after injuries, surgery or laser)
1. You need to use a skin bleaching product such as hydroquinone or Elure at least 4 weeks prior to your treatment.
These products are available for purchase in our office.
2. You should continue to use a skin bleaching product on the treated area for 3-6 months after your treatment.
3. You may need 4-5 treatments instead of 1-2 since we need to perform a more gentle treatment on your type of skin. These treatments will be 6-8 weeks apart instead of 4 weeks apart to reduce the risk of developing more dark spots.
