Fees
We request payment be made at the time professional services are rendered, unless you have made prior arrangements with us to bill your insurance company. We bill many insurance companies; please confirm with us that your insurance company is one of them. Insurance companies frequently set fees that are below customary charges. Patients are responsible for paying any deductibles or balances not paid by insurance.
We are providers for Medicare and most PPO insurance plans. We are not providers for HMO plans.
Cancellations
If you are unable to keep your scheduled appointment, please let us know at least 24 hours in advance, or you may be charged a late cancellation fee.
